Sorry/not sorry if this sounds heartless. This is someone making millions a year in a nationally-sanctioned pyramid scheme that makes money by DENYING necessary care and making patients and doctors jump through hoops. One that employes literally thousands of people without medical training whose sole purpose it is to deny any claim up front and make the patient fight for it. A company that gets rich on the enormous plan costs of people "lucky" enough to qualify for one (I pay out about 1/3 of my paycheck each month for "sponsored" health insurance; it would be 3x as much if I ever have to pay COBRA/unsubsidized rates).
His stooges on the other end of the phone line regularly tell licensed doctors that no, their patient HAS to go on a generic drug rather than Dispense as Written, even though anyone in the field KNOWS that there are very frequently major differences between generics and the OG drugs -- once a generic is available, the FDA (soon to be gutted) almost never does follow up unless there is a major wave of adverse events. Generics may have the same clinical dosages, but the binders, etc are different as are the absorption rates in many cases.
They deny surgery. They dog people for months to deny claims related to an injury followup. When I broke my ankle years ago, I kept getting bothered by calls from an obviously shitty 3rd party call center in India asking me the same questions: Where exactly did I fall and how? Was a business to blame? Was *I* to blame? Was I drunk or on any medications? If it was at someone's private residence, what was the address (I refused to give that to them).
All of this and another reminder: NO health insurance company has EVER seen an actual patient, performed a surgery, did so much as take someone's blood pressure, invented a new drug, helped in a clinical trial (in fact they actively make it incredibly hard for people to participate in them), invented a new medical device, etc. etc. etc. All they do -- ALL THEY DO -- is profiteer off of a necessary service that the entire rest of the fucking civilized world has managed to do better in one form or another.
And don't get me started on "but the US is a kingpin in drug development." That has nothing to do with anything, and guess what most companies now do the bulk of their clinical trial research -- the backbone of drug development -- overseas. Wanna know one reason why? Yep, insurance companies.
One last stat: For every dollar spent on healthcare in the US, how much goes to insurance companies and how much goes to drug development? I'll let you ponder for a second:

Also depending on whose numbers you use, for health insurance: 33-36 cents.
The current for-profit healthcare system is unsustainable in the US in the long run. For my friends who remember Schoolhouse Rock: Remember the cutesy little jingle about "The Shot Heard Around the World" for the start of the American Revolution?
If this is "The Shot Heard Around the Boardrooms" then so be it, and frankly it's a great place to start. I am done, DONE with spending my entire adult working life having to argue about complex medical issues with cretins in headsets so that asshole CEOs can get a few more points on their stock, while they take increasingly huge parts of my paycheck.
